Also known as a patient care manager. Someone who reviews the medical treatment planned by a physician and ensures it plays out to best meet the patient's needs. The social worker coordinates the physician's and patient's desires, insurance approvals, or referrals for rehabilitation, assisted living, and financial assistance. Social workers usually answer any questions or concerns regarding care and often provide confidential counseling for patients coping with difficult diagnosis or treatment and for family and friends bereaving.
